It is alleged that Kelly McGillis was actually assaulted in real life and that is why she agreed to do the film.

I recall reading that originally Kelly McGillis was asked to do the role Foster played, but decided for personal reasons she would rather be the lawyer instead!
This was one of Foster's first major roles since her time in college. Out of the gate with both barrels blazing, she won an Oscar and Golden Globe for her portrayal of Sarah Tobias. She shortly followed-up with a second defining role... Clarice Starling in "Silence Of The Lambs".
This movie was apparently based on a real-life incident that took place in Massachusetts.

The movie is based on a real life crime that took place in a bar in New Bedford, Ma called Big Dan's.
Linda Hamilton, Melanie Griffith, Daryl Hannah, Jennifer Jason Leigh, Joan Cusack, Tatum O'Neal, Molly Ringwald and Jennifer Connelly all apparently turned down the role of Sarah...
Upon seeing a pre-screening of The Accused, Jodie Foster thought her performance as Sarah Tobias was so awful that she immediately began preparing for and taking the GRE's for graduate school. She was prepared to leave her film career behind and focus on academia.
...Until she won the Academy Award for her performance.
